Health Insurance and Billing
All co-payments must be paid at each visit before you see the doctor.

As a courtesy to our patients, we will file insurance claims for services rendered to you by our office. Remember, your insurance plan is an agreement between you and your insurance company. Payment is expected at the time of service for all charges not covered by insurance. Some services we provide may not be paid for by your insurance company. We will, nevertheless, file your claim but require you to sign a waiver before each visit permitting us to balance bill you if the insurance company denies the claim. Payment of the remaining balance is expected at the time you receive your first bill from our office.

We participate in most insurances and other managed care plans. If we do not have a contract with your insurance plan, payment of all anticipated charges are required before you see the doctor and payment of any additional fees is expected before you leave after your visit.

For our patient's convenience, we do accept MasterCard and Visa credit cards and debit cards. We accept checks from established patients but regret that we are unable to accept checks from new patients or for pre-employment or other work related physicals. There is at $22.00 fee charged on all returned checks. Any returned checks not redeemed within 30 days will be prosecuted.
